What are the Documents Required for FSSAI License?

FSSAI- Background

In India, food license is one of the most important licenses which is essential for food business operators (FBOs). Eligible food operators like manufacturer, retailers, transporters, wholesalers, distributors etc. need to obtain food license which is issued by Food Safety and Standards Authority of India, regulated by Ministry of Health & Family Welfare. Its role and responsibility to ensure that all food-related laws are followed by food business operators.

Every food business operators before the commencement of the business mandatorily to get food license which is valid for 1 to 5 years depends upon the choice of FBO’s. and renew from time to time.

There are 3 types of licenses which is allotted by concerned authority FSSAI, after the meet of eligibility criteria:

  1. Basic Registration
  2. State Food License
  3. Central Food License

For obtaining FSSAI Registration or license, it is depending upon annual turnover of the food business and location of the business.

If annual turnover is less than INR. 12 lakhs then, basic registration need to be obtained it is generally for small business operators; or

If annual turnover is more than INR.12 lakhs but up to INR. 20 crores to do business in a particular state then State food license is required to be obtained; or

If annual turnover is more than INR. 20 crores to do business in more than one state then Central food license is required to be obtained.

Documentation is an important concern for getting Food license:

For Basic Registration

  • Form A as prescribed by FSSAI Authority required to be submitted and signed by FBO’s along with supporting documents;
  • 2 Passport size photographs of FBO’s;
  • Identity proof like Pan card, Driving license, Voter id card, Passport, Aadhaar card, senior citizen card;
  • NOC’s by Municipality/ Panchayat, health Department etc. if any;
  • Rent Agreement if any.

For State Food License

  • Form B as prescribed by FSSAI Authority to be filed and signed by Authorized signatory along with supporting documents;
  • Layout plan of the food processing unit;
  • List of directors/partners/members with full name as well as address. However, this is mandatory for companies only;
  • ID proof and address proof for the above persons;
  • List of food category (for manufacturers);
  • Authority letter nominated by the manufacturer to on behalf of him;
  • Proof of ownership or possession of business premises, such as electricity bill or rental agreement;
  • NOC from the municipality or local body if any;
  • For manufacturing and processing unit name and number of equipment’s as per installed capacity and horsepower
  • Partnership deed/ or affidavit of proprietorship in case of a company a copy of MOA and AOA as well.
  • Source of the raw material of meat and meat processing unit and source of milk and milk related products and milk collection center where it is applicable;
  • For hotel industry certificate provided by Ministry of Tourism;
  • For pesticide residue report of water for packaging of water like Mineral, carbonated etc.
  • Food safety management system plan.

For State Food License

  • Form B duly completed and signed.
  • Photo and address proof of Proprietor/Partner/Director(s)/Authorized Signatory.
  • List of Directors with full address and contact details.
  • For processing unit, it is mandatory to mention the dimensions of the operating unit.
  • Authority letter from Director, partner or proprietor.
  • It is also important to mention the list of equipment and machinery along with their number and installed capacity.
  • In case of manufacturers, the list of food category desired to be manufactured should be mentioned.
  • List of food category (for manufacturers);
  • Ministry of commerce certificate for 100 % EOU;
  • IEC documents issued by DGFT;
  • Supporting documentary for proof of turnover;
  • Analysis report of water to be used in the food.
  • Possession proof is important (Electricity bill, sale deed etc.).
  • Partnership Deed / Affidavit / AoA / MoA etc. if available should be submitted.
  • NOCs from the municipality or local body.
  • A canceled cheque
  • Plan for food safety management.
  • Source of the raw material of meat and meat processing unit and source of milk and milk related products and milk collection center where it is applicable;
  • For hotel industry certificate provided by Ministry of Tourism;
  • For pesticide residue report of water for packaging of water like Mineral, carbonated etc.
  • Food safety management system plan.
