Swiss Airline Catastrophe summary

Swissair Flight 111 left from New York on September 2, 1998 at Geneva. The airplane had 215 passengers and 14 crew members. Fifty-three minutes into the flight, the crew smelled an abnormal smell in the cockpit. The crew concluded that the cause of the smoke and odour was a faulty air condition. However, the crew continued the flight until the problem re-emerge. The crew requested for immediate landing but did not manage to reach the destination. It crashed into the sea and killed all on board. TSB conducted a detailed analysis of the probable causes of the fires. The assessment of 98% of its residue indicated that the fire began in the forward, overhead area of the plane. The investigating team determined that an electrical arcing event above the ceiling of the cockpit was responsible for igniting the flammable material on nearby polyethene terephthalate covering on thermal, acoustic insulation blanket. The board concluded that the certification standards of the flammability of materials were inadequate. The investigating team recommended the removal of MPET thermal, acoustic materials and the development of a new flammability testing criteria.
