Swissair Flight 111 left from New York on September 2, 1998 at Geneva. The airplane had 215 passengers and 14 crew members. Fifty-three minutes into the flight, the crew smelled an abnormal smell in the cockpit. The crew concluded that the cause of the smoke and odour was a faulty air condition. However, the crew continued the flight until the problem re-emerge. The crew requested for immediate landing but did not manage to reach the destination. The board concluded that the certification standards of the flammability of materials were inadequate. The investigating team recommended the removal of MPET thermal, acoustic materials and the development of a new flammability testing criteria.
