ESSENTIAL REQUIREMENTS FOR OBTAINING THE WHOLESALE LICENSE 

Following are the essential requirements for obtaining the wholesale drug license:

Technical staff- The Technical staff can varies from wholesale technical staff to retail staff. The sale of the drugs by the wholesale shall be made either in the presence of the competent person who has of at least a year experience in dealing with the drugs along with the graduation degree approved by the department of drug control. Also, he can be a person, who has passed S.S.L.C with 4 years’ experience in dealing with drugs. On the other hand, the sale of the drug by retail must be in the presence of the registered pharmacist approved by the department.  

Area- The minimum requirement to start with a pharmacy, medical shop or wholesale outlet is 10 square meters or the retail or the wholesale shop can be 15 square meters.

Storage Facility –The shop must have the facilities of a refrigerator or the air-conditioner on the premises.

It is mandatory that once the drug license is obtained, the same should be displayed on the premises of the area in the case of future inspection.

• Covering letter signed with the name and designation of the applicant.
• Challan of fee deposited.
• Declaration form in the prescribed format.

• Freezer purchase bill with the address.
• Ownership or rent agreement deed of premises.
• The electricity bill for the premises.
• Firm registration receipt in case of a private limited firm.

• Affidavit of the proprietor on stamp paper of 20 Rs.
• Educational certificate copies of the proprietor (Self-attested)
• Self-attested copies of Identity proof of proprietor ( Domicile/ Driving license/ Voter ID card)
• Affidavit of Regd. Pharmacist.
• Educational certificate copies of Regd. Pharmacist (Self-attested)
• Self-attested copies of Identity proof of Regd. Pharmacist ( Domicile/Driving license/ Voter ID card)
• Blueprint of a Plan layout of the proposed area/ premises.
• The electricity bill for the proposed area/ premises.
• Copy of Refrigerator bill.
• Rent agreement ( 50 Rs. Stamp paper)
• Ownership document of the premises including the copy of tax receipt of proposed area/ premises.
• Covering letter.
• Photo 5-5 each of Regd. Pharmacist and proprietor.
